Problems With the Temporomandibular Joint and Chewing Muscles

Jaw pain, clicking when opening the mouth, headaches, facial tension, or limited mouth opening are common complaints linked to the temporomandibular joint. At first glance, it might seem the problem lies directly in the joint. The real cause is often overloaded chewing muscles and their increased tension.

What Is the Temporomandibular Joint?

The temporomandibular joint is a paired joint connecting the lower jaw to the skull. It lets us speak, bite, chew, swallow, and yawn. It’s one of the most heavily used joints in the human body, working together with several powerful chewing muscles with every movement. For the joint to function properly, the joint itself, the surrounding muscles, and the nervous system all need to be in balance. When that balance is disrupted, it can cause pain, restricted movement, or unpleasant sounds when opening the mouth.

Why Does Tension Build Up in the Chewing Muscles?

One of the most common causes of these problems is chronically increased tension in the chewing muscles. Many people don’t even realise that during the day they unconsciously clench their teeth or tighten their jaw. We see this more often in people who are more sensitive to stress, tend to overanalyse things, dwell on problems, and stay in a state of heightened alertness for long periods. In these people, the sympathetic nervous system is more active, preparing the body for strain — and one expression of that can be exactly this unconscious jaw clenching and overloading of the chewing muscles. Symptoms can also get worse in winter. When we’re cold, we naturally raise our shoulders, tense our neck muscles, and often clench our teeth more too. This protective response helps the body retain heat, but it also increases the load on the jaw joint and surrounding muscles.

What Are the Most Common Symptoms?

Disorders of the temporomandibular joint and chewing muscles can show up in different ways:

  • jaw or facial pain
  • headache
  • clicking or popping in the joint
  • limited or painful mouth opening
  • headaches, especially around the temples
  • pain radiating into the ear with no signs of an ear condition
  • increased tension in the neck and shoulders

Because the jaw is closely connected to the cervical spine and neck muscles, symptoms are often present outside the joint itself as well.

How Can Physiotherapy Help?

In most cases, these problems are caused mainly by muscle overload and a breakdown in their coordination. Manual physiotherapy can effectively reduce that tension, restore proper joint function, and ease the pain.

Treatment may include:

  • manual release of the chewing muscles
  • trigger point therapy
  • post-isometric relaxation
  • mobilisation of the temporomandibular joint
  • treatment of the cervical spine and soft tissue
  • retraining of correct jaw movement and an individual exercise programme

Therapy also involves looking for the underlying causes of the overload — for example unhelpful movement patterns, posture, or long-term stress.

The Role of Awareness and Managing Stress

For many patients, simply releasing the overloaded muscles isn’t enough. Just as important is learning to notice the moments during the day when you unconsciously clench your teeth or tighten your jaw. A very effective part of treatment is therefore practising conscious jaw release, breathing exercises, and mindfulness techniques that help lower overall muscle tension. At rest, the lips should be lightly closed, the teeth should not touch, and the tongue should rest gently against the roof of the mouth. Combining manual therapy, active exercise, and greater body awareness tends to bring the best long-term results in most cases and significantly reduces the risk of symptoms returning.
